The South Australian Freedom of Information Act (1991) allows you to access your medical records on request. Your records include present and past spectacle and contact lens prescriptions, referral letters and examination notes.
How do I request access to my medical records?

Print and fill out the form. Send the form to us through fax (8232 1600), email (wakefield@innovativeeyecare.com.au) or post (300 Wakefield Street, Adelaide SA 5000).
What fees apply?
In accordance with the South Australia Freedom of Information (Fees and Charges) Regulations 2018.
- $38.25 fee per application for medical records
- $0.20 per page of records printed
- $8.25 per page in a summarised report
- Cost of postage
